Pharmacist I

a)To Provide a safe and efficient delivery of pharmacy service (dispensing/counseling) in the hospital. b)To co -ordinate activitvies in his/her scope of work to align with the objectives of pharmacy department Key accountabilities a) Review medication orders for appropriateness, process drug orders, dispense medication to patients and supply drug stocks to nursing units. b) Counsel patients (inpatients/outpatients) on their medication c) Provide drug information to doctor ,nurses and patients d) Perform extension hours, night duties and on -call duties as assigned by duty roster (if applicable) e) Assist in the preparation of ad -hoc items required on prescription orders f) Adhere to legal and hospital policy requirements. g) Participate in continuing professional education(CPE) h) Assist in the training and supervision of pharmacy assistants and technicians. I) Participate in quality assurance activities (i.e ward inspection/expiry checks) as assigned. j) Perform medication -therapy related duties (i.e supply and distribution of controlled drugs , adverse drug reactions reporting, drug usage monitoring, etc. k) Participate in medication reconciliation rounds and specialized counseling (warfarin and inhalation techniques counseling) l) Assist in providing clinical experience to pre -registration pharmacists and students on attachment (if applicable) m) Maintain records relating to drug supply, storage and distribution as required by hospital and legal regulations(i.e JCI, MOH standards. n) Perform the function of purchasing pharmacist on a rotational basis (if assigned/if applicable) o) Assist in reviewing and updating procedure/protocols/intranet materials on a regular basis. p) Participate in approved pilot schemes/new services (if applicable) q) Represent pharmacy in various hospital projects as assigned by in -charges (if applicable)

a) Bachelor of Science in Pharmacy from an accredited college of Pharmacy or university b) Registered with Singapore Pharmacy Council c) At least 2 years of relevant work experience This job position is added by Jooble, a Job Agency in Singapore.
